Low Dose Naltrexone And Hashimotos

Have you heard of Low Dose Naltrexone, or LDN? Since the beginning of my Hashimotos journey, this medication has been on my radar, and in recent years, I have learned so much more about it.
In 2015, I conducted a survey of over 2000 readers with Hashimotos, asking for the most helpful interventions in their health journeys.
One reader wrote: Low Dose Naltrexone changed my life for the better. If I had not added that to my regimen, I would still be suffering from nerve pain and uncontrollable allergy symptoms, in addition to the problems I have from Hashimotos.
While only 38 percent of those that tried Low Dose Naltrexone reported feeling better, those who did see benefits, had some spectacular results. Forty-eight percent were able to reduce their thyroid antibodies, 61 percent saw an improvement in mood, 66 percent had more energy, and 40 percent saw a reduction in pain.
I have personally seen this medication change peoples lives and wanted to share some more information on it, in case you have hit a plateau in your healing journey.

Interview With Shannon Garrett
I also interviewed Shannon Garrett, RN, who is a Functional Nurse Nutritionist, LDN Nurse Educator, and founder of Holistic Thyroid Care for The Thyroid Secret docuseries. She works with clients with Hashimotos, and helps them use LDN as a tool. She also shares my root-cause philosophy, and I want to share with you some of the pearls I learned from our conversation.
Shannon has had her own journey with Hashimotos and saw 9 doctors before getting a diagnosis and spent many more years exploring her own root causes. When she added LDN to her protocol, she reported an improvement in her quality of life. And now, she helps others to do the same.
The most important point I learned from Shannon is that LDN wont work unless certain underlying issues are also addressed. It is not a stand-alone treatment. You will need to address gut imbalances, follow an anti-inflammatory diet, address nutrient deficiencies, and support the adrenals. She explained to me that Candida is the most important factor that will block LDN from working, and that Candida is always an issue with LDN because it is a natural part of the immune system. LDN can have a down regulating effect on the immune system and cause yeast and other opportunistic pathogens to thrive, if they are not addressed. Shannon recommends getting stool testing, treating Candida and avoiding sugar, if you are considering using LDN.
The Effectiveness Of Ldn
OK, so what conditions is LDN effective for? There are actually two ways to answer this question. The first is what the scientific literature shows, and then the second is what clinical and anecdotal experience of clinicians that are working with LDN shows. Theres definitely research out there on LDN, but its still somewhat limited, and I think clinical and anecdotal experience is further ahead in terms of the breadth of conditions that LDN is being used for and the experience of how effective it can be for those conditions. The studies are also still usually relatively small sample size, not always randomized, not always double-blinded. Part of the reason for this is theyre probably not that well-funded because low-dose naltrexone is off patent, and that means that drug companies dont stand to make a killing on selling LDN, and its unlikely that a whole lot of money is going to be put into it for that reason.
One of the advantages of LDN as a therapy is that its low cost. Its off patent, as I said, which means typically you can get it for about 40 bucks a month, 35 or 40 bucks a month from a compounding pharmacy.
We still need more research. I mentioned that the research we have already is somewhat limited, so we need more research.

How To Prepare Low Dose Naltrexone
This page describes the steps to take to prepare a solution containing one milligram of nalrexone per milliliter.
Since there is no recognized use of naltrexone for off-label applications, there are also no officially recognized doses for any off-label conditions. The instructions below assume you will prepare a dilution of naltrexone in a consistent manner and take a consistent ml dose regardless of small measurement errors by using common household utensils.
Dilution Versus Compounding Pharmacist

Not very many doctors prescribe low dose naltrexone. Of those who do, many write a prescription for a compounding pharmacist to prepare the prescribed dose. While there are a few on-line pharmacies who prepare low dose capsules at a reasonable price, most compounding pharmacies do not have the economy of scale to prepare the prescription at a low price.
If your doctor will listen to you, ask for 50 mg tablets, which cost around $30 for 30 tablets. Because the standard dose for naltrexone is 50 mg daily by mouth, the instructions on the prescription may read, take one tablet by mouth once daily Sometimes the pharmacists express concern because they want to give you instructions about taking a new prescription and the part about titrating throws them off unless they have previous experience with LDN.
There are websites that have good information about how to obtain a prescription if your own doctors are hesitant. Lowdosenaltrexone.org is one such site.
A single 50 mg tablet costing about one dollar will last 11 days when a person is taking 4.5 mg daily. In practice the last dose may have a lot of the excipient in it and there is no great loss in discarding it.

Compounding Pharmacies And Low Dose Naltrexone
The FDA has not approved low dose naltrexone treatment for anything other than substance abuse, including opioid addiction and alcoholism. But in lower doses, naltrexone could beneficial to patients suffering from many different medical conditions.
Because the FDA has not approved naltrexone for off-label uses, the only way to obtain naltrexone in low doses is to get your prescription filled through a compounding pharmacy such as The Healthy Choice. Compounding pharmacies differ from standard pharmacies in that we have the ability to develop drugs that are designed to meet your specific needs.
For example, if you were prescribed 2 mg of low dose naltrexone, you would need a compounding pharmacy to develop that specific dose of the drug, as naltrexone is normally available only in 50 mg tablets. Additionally, if you are unable to take pills orally, there are other options that may be available to you.

Types Of Low Dose Naltrexone
- Capsules – It is critical to remember that Low Dose Naltrexone should not be prescribed in SR or slow release form. The capsules should also be filled with a neutral, inactive filler such as lactose or avicel.
- Sublingual Drops – Occasionally the drug will need to be purchased as a solution in distilled water. In these cases, 1 mg/ml will be dispensed in a medicine dropper. The solution must be refrigerated.
- Cream – Low Dose Naltrexone can also be administered in a 0.5 mg /ml dose as a cream that’s applied to the skin. This is most helpful for children or patients who have allergies to colors, excipients, or flavorings found in other forms of Low Dose Naltrexone.
Who Is A Good Candidate For Low Dose Naltrexone
Patients suffering from mental health conditions, chronic pain, inflammation, autoimmune disorders, and more may benefit from the use of low dose naltrexone. But that doesnt mean everyone suffering from these conditions is a good candidate for LDN.
For example, patients suffering from hepatitis or liver failure would not be good candidates for low dose naltrexone treatment. Individuals taking Wellbutrin, Suboxone, hydrocodone, morphine, Cymbalta, ibuprofen, or Methadone may also not be good candidates for LDN due to the high-risk drug interactions.

Where To Find Ldn And How To Take It
LDN is not a commonly prescribed medication and is only available as a prescription. It needs to be compounded into lower doses by a professional compounding pharmacy. Luckily, even without insurance coverage, this medication is available in generic form and is very affordable, usually costing between $15-$40 per month.
It may not always be possible to convince your doctor to prescribe LDN. On my quest to try LDN, I talked to my local compounding pharmacist to figure out which doctors in my area were knowledgeable about prescribing LDN. Now, I recommend for physicians to speak with an LDN-trained compounding pharmacist to learn more.
There have been concerns about compounding medications not being accurate throughout, so I always make sure people use pharmacies that have PCCA starting materials. They dilute the ingredients, which makes it much harder to make an error with the dose. The key is to start out low and adjust according to what your body needs. Doses of 1.54.5 mg per day are usually recommended.
What Are The Benefits Of Low Dose Naltrexone

Accuracy Of Drug Substance Contained In Capsules And Tablets
Naltrexone Tablets 50 mg
The United States Pharmacopeia is a compendium of information about drugs that establishes quality standards. It refers to the active ingredients of a medication as the drug substance. Capsules and tablets containing the drug substance are required to adhere to standards of purity and quantity. The margin of error for the amount of drug substance in each tablet is very small. A tablet of naltrexone, whether generic or brand name, contains 50 milligrams of naltrexone plus or minus a very small amount. Everything you do to prepare the low dose used for Low Dose Naltrexone purposes will introduce small and inconsequential errors in the final amount contained in one milliliter of distilled water.
What If My Dosage Is Too Low Or Too High
When you first start taking naltrexone, you wont know how much of the medication youll need to take every day. The best thing to do is to keep track of how much youre taking and, working with a qualified health professional, start with a dosing schedule that starts out low and gradually increases over time as your dose builds up and you feel positive effects.
The problem is, unlike most medications, there are no clinical trials that have set the standard for the right dosage of LDN for a clinician to prescribe. So it is impossible to know if you are taking the right amount or if you should take more except by the process of trial and error.
For those using LDN for depression, start with the smallest dose possible and then gradually work your way up until you feel less depressed. Be honest with yourself about your condition and stop at the appropriate level, knowing this drug tends to work best at very small dosage levels.
Most experts agree with the patient consensus that each person is different and you have to experiment to find your optimal dose. To go through this process and finally discover what worked best for you, its suggested to start with 8.5 mg, try 10mg the next day, and then 12.5 mg the day after that, increasing in increments of 0.5 mg per day each time until you find the optimal amount.

Prescribing Low Dose Naltrexone
It isnt difficult to find information about the use of small amounts of naltrexone hydrochloride for the treatment of neurological disorders, particularly multiple sclerosis.
Naltrexone has been available for decades as a treatment for a handful of addictive disorders. It proved to be less effective than hoped and soon fell into disfavor. However, one New York physician, Dr. Bernard Bihari, noticed that when his AIDS patients used naltrexone for their addictions, they also experienced marked improvements in the neurological symptoms associated with AIDS.
For a variety of reasons, Dr. Bihari tried lower doses of naltrexone and observed that his patients still had symptoms improvement, even when doses were 3mg . There appeared to be a connection between autoimmune symptoms and the use of naltrexone in low doses. Cancer patients also showed improvement. People who suffered from multiple sclerosis were offered this low dose of naltrexone and also reported improvements in all symptoms associated with that disease.
Numerous compounding pharmacists were in contact with Dr. Bihari and began preparing a 3mg strength of naltrexone. Initially, we used commercially available naltrexone tablets . Because the drug had been around longer than its patent life the pure powdered form of naltrexonehydrochloride became available to compounders.

Accuracy Is Not Critical
The dose of naltrexone you take to treat off-label conditions is determined empirically. Usual doses taken for autoimmune disorders and cancer range from 1 mg. to 10 mg. The most common published average dose is 4.5 mg, which means very little because arriving at the 4.5 mg dose is based on symptoms and most people cannot tell the difference between symptoms at 4 mg or 5 mg, although many can tell when a smaller dose works better than a larger dose, e.g. 2 mg vs. 4.5 mg.
We are not recommending any doses here we are just describing how to prepare a solution containing close to 1 mg. per ml. As long as you use the same measuring paraphernalia, your dilution will remain essentially the same over the extended time that you take it. Most people find a dose that works and stay there.
